Some evidence suggests the liver can package microplastics into bile and excrete them into the digestive tract for removal [4, 21]. Bile acid transporters may assist in this effort, pushing out microplastic debris alongside other waste products [4].

However, this process isnโ€™t efficient. Many microplastics, especially nanoplastics, are simply too small or too chemically complex to be cleared out quickly. Because of this, the liver works overtime, managing a growing toxic burden with limited tools [4, 21].

Researchers are still mapping the precise pathways that microplastics take within the liver. However, itโ€™s apparent that microplastics induce inflammation in the liver and present unique challenges in comparison to other toxins. Additionally, instead of being filtered and flushed, many particles linger and accumulate.

How Microplastics in the Liver Cause Damage

The production of ROS creates inflammation. The liverโ€™s Kupffer cells release cytokines like IL-6, IL-8, and TNF-ฮฑ, triggering widespread tissue irritation and immune activation. Left unchecked, this inflammation leads to liver fibrosis and cell death, either through apoptosis or necroptosis [2, 15, 23โ€“26, 35โ€“37].

Microplastics also mess with bile acid metabolism, slowing bile flow, reducing volume, and impacting pathways that regulate cholesterol and glucose [4, 38].

The Molecular Pathways Behind the Damage

Scientific research has shown that microplastics impact several critical molecular pathways.ย 

  • ROS-Mediated MAPK Signaling: This stress-response pathway ramps up inflammation and cytokine production [28].
  • PTEN/PI3K/AKT/Autophagy Axis: Disrupted by polystyrene microplastics, leading to programmed cell death and liver injury [26].
  • cGAS/STING Pathway: Associated with the onset of liver fibrosis in response to microplastic buildup [35].
  • Nrf2 Antioxidant Response: Sometimes activated in a last-ditch attempt to neutralize oxidative stress [27].

These molecular changes can potentially lead to long-term liver damage.ย 

What Makes Some Microplastics Worse Than Others?

Studies have shown that various factors influence microplastic toxicity in the liver. For example, the size and polymer type play a role: Smaller particles (especially nanoplastics) penetrate deeper into tissues and are harder to excrete. Polymers like polystyrene and polyethylene show stronger effects [2, 6, 15, 26]. Other factors include aging and weathering. Older plastics shed more toxins and become more chemically reactive [33, 39]. Moreover, microplastics often carry other contaminants, like heavy metals, amplifying their toxicity [14, 35]. Finally, microplastic toxicity can be influenced by lifestyle decisions. High-fat diets have been linked to less healthy livers, making the liver more vulnerable to microplastic-induced damage [9, 35].
